Create a patient from search

Create a patient chart from search or from the Patients page.

Enter the patient's name. Add the date of birth and sex if you know them.

  • On a session, click Link a Patient. The search says "Search patients by name, DOB, or ID..." Create new patient appears at the bottom with your text.
  • In Patient Name on Write a note, Upload audio, or the recorder, select Create Patient under Add New Patient.
  • On the dashboard, type in Find a patient (optional). If the search says No patients found, select Create Patient.
  • On Patients, search for a patient. If no charts match, select Create Patient next to Clear filters.

Click Create Patient to open Confirm New Patient. The dialog says "Review the patient details before choosing an existing patient or creating a new record." While it reads your text, it shows Preparing patient details...

Questions

The Questions section can ask up to 3 questions. The text says "Answer what you know. Skip leaves that field unset."

Each question has suggested answers, Other, and Skip / Ignore. Click Other to open Enter another answer. Click Skip / Ignore to leave the field empty. Click Submit answers when you finish.

Extracted details

The Extracted details section shows First name, Middle name, Last name, Date of birth, and Gender. An empty field says Not provided. Dates use MM/DD/YYYY.

If questions are open, the details appear in a one-line summary. If there are no questions, the details are open.

If you enter one date, such as 04/15/1988 or 04/15/88, the form shows it as MM/DD/YYYY. For a two-digit year, it uses the latest year that is not in the future. If it cannot use the date, the dialog asks "What is the patient’s date of birth?"

If your text matches a label, Gender shows Male, Female, Intersex, Unknown, Other, or Declined to answer. It shows m or male as Male. It shows f or female as Female. Other text stays as you entered it.

Read the name before you continue.

Click Revise to open Add or correct details. Enter a correction, such as "The date of birth is 04/15/1988." Click Apply revision to update the draft. Click Cancel revision to close the box.

If the first or last name is missing after you answer the questions, the dialog says First and last name required. It also says "Revise the draft before creating this patient."

Possible duplicates

The Possible duplicate patients section lists charts that look like the same person. Each result shows a name, DOB, and Choose existing. The text says "Choose an existing patient if one matches this person."

Click Choose existing to use a chart from the list. To make a separate chart, select Create a separate patient despite these matches. The text says "I reviewed the possible duplicates and confirmed this is a different person." The Create button stays off until you select the box.

Create or cancel

Click Create to add the chart. You must submit the questions and enter a first and last name before you can click it.

Click Cancel to close the dialog without a new chart.

If you start from Link a Patient, the chart links to the open session. If you start from Write a note, Upload audio, or the recorder, the chart appears in Patient Name. The page stays open. If you start from the dashboard, the chart appears in Find a patient (optional). If you start from an empty Patients search, the chart opens.

If Nextvisit cannot prepare the details, the dialog says Unable to continue. Click Retry.

From the patient list

Click New patient in the header of Patients. This opens New Patient and shows "Add a new patient to the system."

Administrative Sex offers Male, Female, Intersex, Unknown, Other, and Declined to answer.

You can use Fill from patient records to enter form data. Select Upload PDF or Paste text. Then click Fill patient form. To upload a PDF, drop the file or click to select it. The PDF can be 10 MB or less. The form says it saves the PDF in the patient's Documents. When it finishes, the form says "Records added. Review the populated fields before saving."

If Nextvisit cannot attach the records, the dialog stays open. It shows "The patient records could not be saved." Nextvisit does not save the chart.
