Chart organization
Review duplicate patients
Find matching charts, keep one record, merge or archive the rest, and mark groups that are different people.
Open Duplicate Patients under Apps. These steps apply when Duplicate Patients appears in the Apps group in the sidebar.
The page scans your workspace for charts that look like the same person. Pick the record to keep, then merge the rest into it. Mark groups that are different people as not duplicates.
Review groups
The scan runs when the page opens. Rescan runs it again.
Each group shows a confidence badge: Likely duplicate, Possible duplicate, or Worth a look. Other badges show why the charts were grouped: Same name, Nickname, First and last swapped, Similar spelling, Same date of birth, or Different date of birth.
Search by name or ID. Filter Likely, Possible, or Worth a look. Clear removes the search and that filter.
Each row shows the name, record ID, date of birth and age, visit count and last visit, email, phone, provider, and the date the chart was added. Open chart opens that chart in a new tab.
If the dates of birth differ, the group shows: The dates of birth differ. Open both charts before merging. If a date of birth is missing: A date of birth is missing, so the match is based on the name only.
The record with the most visits is listed first and selected to keep.
Keep and merge
On a group that is still under review:
- Select the circle next to the chart to keep, or open the row menu and choose Keep this record. That row shows Keep.
- Other rows show Merge and archive.
- In a group of three or more charts, the row menu Leave out of merge marks a chart Left out. Include in merge puts it back.
The footer states how many records will move into the kept name. Choose Merge N into [name].
Confirm Merge these patients? The dialog names the kept chart and each chart that is merged in and archived. Visits, notes, documents and other records move to the kept patient. Blank details on the kept record are filled in from the merged records. A merge cannot be undone.
Confirm Merge patients.
After a merge, open the kept chart. Medications, conditions, and allergies from both charts appear there. If both charts listed the same medication, both entries can appear.
Mark as not duplicates
Choose Not duplicates when the charts are different people. Those records will not be paired again.
Turn on Show groups marked as not duplicates to see them under Marked as not duplicates. Undo puts the group back on the next scan.
Archive without merging
The row menu Archive this record opens Archive this patient? That patient is hidden from your patient list. Nothing is merged. Confirm Archive patient.
To restore an archived patient:
- Open Patients.
- Select Archived.
- On that row, choose Restore patient.
When the list is empty or a scan fails
| What you see | What to do |
|---|---|
| No duplicates to review | No matching names were found. Rescan after adding or importing more patients. |
| No groups match your filters | Clear filters. |
| Scan failed | Retry scan. |
| Duplicate Patients is not in Apps | Ask the workspace owner whether Duplicate Patients is enabled for the workspace. |
