Onboard a provider

Set up the account, provider identity and payer transactions.

Owner or workspace administrator
Gather before onboarding Where it is used
Rendering provider The provider must belong to the selected workspace.
NPI and tax ID The provider form requires a 10-digit NPI and a 9-digit tax ID.
Taxonomy code Identifies the provider specialty on eligibility requests and claims.
Billing name and address Include street, city, state and ZIP code.
Service location Select an active location in the same workspace.
Claim.MD payer ID Use the payer’s clearinghouse identifier for the required transaction.
Enrollment evidence Retain the payer reference, verification source, date and decision.

Step by step

  1. Select the workspace and environment.

Open Insurance → Setup. Choose Production for live work or Test environment for training. Confirm the environment before entering patient or account information.

  2. Save the clearinghouse connection.

Under Add connection, choose Claim.MD or Stedi and the matching environment. Enter the Claim.MD account key or Stedi API key. For Stedi, select Enabled Stedi features. A test connection also requires the synthetic-record confirmation. Enable Enable external transaction writes when you are ready to send requests, then select Save connection. The connection form labels the test environment Test account.

  3. Refresh payer capabilities.

Select Refresh payer directory on the saved connection. Resolve any connection error before continuing. Saving a key alone does not verify connectivity.

  4. Add the rendering provider.

Select Add provider. Choose the provider and connection, enter the NPI, tax ID, taxonomy, billing name and address, then select Save provider.

  5. Complete payer enrollment by transaction.

Under Start payer enrollment on a Claim.MD connection, select the provider, enter the Claim.MD payer ID and choose Eligibility, Claims or Electronic remittance. Select Start enrollment, open the returned enrollment link and complete the payer instructions. For Stedi, import history from the connection, then use Record verification.

  6. Record the verified payer decision.

Select Record verification on the payer record. Enter the status, payer reference, source and verification date, confirm the evidence, then select Save verification. Repeat for each required payer and transaction. Use Enrollment not required only with documented evidence.

The connection is saved, the provider identity is present, and each required payer transaction has a recorded decision and verification evidence.
Read the payer’s transaction status

A saved provider identity or an enrollment link does not confirm that a payer transaction is active. Review the recorded payer decision and verification evidence for each transaction.

Add or correct a policy

Keep coverage order, dates and subscriber details accurate.

  1. Open the patient’s policies.

From Eligibility → Check coverage, select the patient. Open Patient policies to add or correct coverage details, then return to the coverage check.

  2. Record the policy and its order.

Choose the correct payer and identifier. Enter the member ID, group information, effective dates and primary or secondary priority.

  3. Check patient and subscriber identity.

For a dependent, enter the subscriber’s name, birth date and relationship separately from the patient’s details. Confirm spelling and dates against the policy information.

  4. Save and request fresh eligibility.

Return to Check coverage, reload policies if needed and select the saved policy. A policy edit creates a new verification context; an earlier response remains historical evidence.

The saved policy is available for a new check, and its patient/subscriber details match the source you reviewed.
After a policy change

Request coverage for the updated policy. Previous checks and sent claims retain the details used at the time. A corrected policy does not rewrite that evidence.

Check and understand coverage

Separate the request outcome from coverage and benefits.

  1. Choose the patient and policy.

Open Eligibility, select Check coverage and choose the patient’s policy. If policies fail to load, use Reload policies or Retry before continuing.

  2. Set the visit context.

Choose the clearinghouse connection, rendering provider, service location and service date. Enter the Service type code and an optional Procedure code. The form identifies MH as mental health and 30 as health plan coverage.

  3. Send and wait for the recorded result.

Submit the coverage check. The request is queued and progresses separately from coverage. A repeated request with the same current context reuses its existing result or pending request.

  4. Review the service-specific evidence.

Open the result and review coverage, payer messages, reference, dates and every relevant benefit row. Match the service, network, individual or family level, and benefit period. Record a follow-up when required information is missing.

Coverage status

Active

The payer returned active coverage for the response context.

Your next action

Review the applicable benefit rows. Confirm the provider’s network participation separately, and review any authorization requirement.

Read this field What it tells you
AI benefit summary Estimated visit cost, Copay, Deductible remaining, and Coinsurance when those values are returned. Use View payer sources and Full payer response for the retained evidence.
Service and procedure The benefit must apply to the planned service.
Network A returned network category describes the benefit. Verify that the rendering provider participates in that network.
Individual / family Keep individual and family accumulators separate.
Period and dates A remaining amount, annual total and per-visit amount answer different questions.
Amount and percentage A returned $0 is a value. A missing amount remains unknown.
Authorization, limits and messages Read related payer conditions before preparing the visit or an estimate.
Coverage and final payment

Use eligibility to prepare the visit. Final payer adjudication determines payment and patient responsibility. Verify the provider’s network participation and any authorization separately.

Prepare for the visit

Work a queue of visits that need current coverage information.

  1. Add the visit to preparation.

Open Overview → Add visit. Choose the patient, policy, connection, rendering provider, visit date and time, service type, optional procedure and service location.

  2. Choose automatic checking if appropriate.

Select Run scheduled eligibility checks before this visit to opt in. Save with Add visit to preparation.

  3. Work the preparation queue.

Filter by date or provider. Select the visits you want to verify and choose Check coverage on Visits to prepare. Each visit uses its saved policy, provider and service context.

  4. Resolve changes before the visit.

Review incomplete or changed coverage, authorization requirements and patient estimates. Use Cancel visit for a cancelled preparation item; reserved authorization usage is released.

Selected visits reference their saved coverage context, with follow-up visible when information needs attention.
This queue is maintained in Insurance

Add visits manually and keep them current. Opted-in checks run for upcoming visits within 2 days when scheduled processing is operating. External appointment-calendar synchronization is not included.

Prepare and submit a claim

Review the encounter, billing details and exact submission.

  1. Prepare the encounter.

Review and sign the encounter before submitting an insurance claim. Confirm the patient, clinical documentation, diagnoses and services performed.

  2. Create the draft.

Open Claims → Create draft. Select the patient and Encounter, then choose the insurance policy, rendering provider and service location. You can also start from the encounter’s insurance claim action.

  3. Complete the billing details.

Enter the service date, place of service, patient billing address and assignment, release and accident responses. Add diagnoses and service lines with procedure codes, modifiers, units and charge per unit. Match each diagnosis pointer to the numbered diagnosis it references.

  4. Review supporting material.

Add authorization numbers where required. If a payer needs an attachment, review the PDF and disclosure authorization before uploading it under Supporting documents.

  5. Save, then open the final review.

Save the draft. An authorized submitter selects Review submission, resolves the field errors and verifies the displayed provider, policy, lines, diagnoses, total and environment.

  6. Submit the reviewed version and follow its events.

Confirm the review and submit. Check Submission attempts and Payer events. Refresh status reloads stored information; the administrator can request clearinghouse polling from Remittances.

Read each event in order

A submission receipt confirms transport. Payer acceptance and adjudication are later evidence. A rejection, denial or unknown result needs its own follow-up.

Correct, void or appeal

Open the claim and select Correct or void. The sheet title is Correct or void claim. Replacement and void submissions use the Payer claim control number.

On Claim.MD claims, Prepare hosted appeal opens the available hosted workflow. Follow the payer instructions and retain the reference and evidence. Stedi claims do not show hosted appeals, secondary insurance, or supporting documents.

Prepare secondary insurance

Review the primary adjudication and policy order first. Under Secondary insurance on a Claim.MD claim, choose the secondary policy and confirm the review.

Select Create secondary draft, then review its coordination-of-benefits details and submit through the normal review flow.

Attach a payer document

Open Supporting documents on the claim. Choose the attachment type and a PDF. The app accepts up to 10 PDFs per claim version, each up to 16 MB. Confirm review, authorization for payer disclosure, and exclusion of separately maintained psychotherapy notes before uploading.

Review the attachment status and any upload error before submitting the claim. The attachment stays associated with the reviewed claim version.

Reconcile payments and balances

Review payer advice before posting it to the ledger.

Owner or workspace administrator
  1. Retrieve and open the remittance.

Open Remittances. An authorized reconciler chooses the connection and selects Poll for responses. Open the remittance to review its payer and payment reference.

  2. Review exceptions and claim matches.

Compare the patient, claim and service-line evidence. Where matching is incomplete, use Match claim and lines, select the correct claim and line mapping, confirm review and select Save match.

  3. Reconcile money and adjustments.

Review payer payment, patient responsibility, adjustment groups and codes, and any provider-level adjustments. Resolve unmatched or unbalanced advice before posting.

  4. Verify the bank payment and post.

In Posting preview, enter the Verified bank payment reference, confirm payment verification and review, then post. The remittance and ledger retain the posted result.

The reviewed remittance posts once, with its source evidence and resulting ledger entries retained.

Record or reverse a manual transaction

  1. Open the claim and choose Record transaction.

Select the transaction type and responsible party. Enter the amount, transaction date, reference and description. Identify an advance copay or an intentional overpayment when applicable.

  2. Review the recorded balances.

Open Ledger for the posted entry. Payer balance, patient balance and money received represent different amounts.

  3. Reverse an incorrect manual entry.

An administrator selects Reverse entry in Ledger and records a reason. The reversal retains the original transaction. For an ERA posting, use Reverse remittance on that remittance.

Amount Meaning
Payer balance The remaining receivable assigned to the insurer.
Patient responsibility The amount assigned to the patient by adjudication.
Patient balance The remaining amount assigned to the patient after recorded transactions.
Collections Recorded money received, adjusted by refunds and reversals.
Nextvisit service fees Separate from patient and insurer transactions. Use your configured pricing; this guide does not quote fees.
Read report dates

Reports use service dates for claim cohorts. Ledger filters use transaction dates. Match the environment and date basis before comparing totals; test and unverified legacy records are excluded from trusted production totals.

Track authorizations and estimates

Retain payer decisions, approved usage and the basis for patient estimates.

Track an authorization

  1. Record the payer requirement or decision.

Open Authorizations → Record authorization. The page heading is Authorizations & estimates. Select connection, policy, provider and service. Enter the payer decision, effective dates, unit type and units authorized.

  2. Retain the source and next action.

Record the authorization number when approved, payer reference and decision evidence. Set the next action and decision due date for work still in progress. Update the record with Update decision as the payer responds.

  3. Reserve and release visit usage.

Select Manage usage, choose the visit, operation and units, then Save usage. Reservations must fit the approved provider, policy, service, dates and remaining units. Release unused reservations when plans change.

  4. Consume units for completed visits.

Consumption requires a completed visit with reserved units. If the visit is not recorded as completed, ask the workspace administrator to confirm its status before consuming units.

Create an estimate

  1. Select the visit and payer evidence.

In Authorizations (page heading Authorizations & estimates), choose the visit and a current payer response. Select the benefit rows that apply to that visit.

  2. Document the calculation inputs.

Enter the Allowed amount and its source, the network and Provider network evidence. Choose the cost-sharing calculation and record assumptions for the reviewer.

  3. Calculate and resolve missing information.

Select Calculate estimate. If the result needs review, resolve the missing or conflicting inputs. Keep returned zero values; do not replace unknown benefits with assumed amounts.

  4. Approve and record completed delivery.

An authorized reviewer selects Approve estimate. After staff deliver the approved estimate, select Record delivery and enter the channel and recipient or delivery reference. This records delivery already performed; it does not send an email or text.

An estimate records assumptions

An approved estimate is based on the selected evidence and documented allowed amount. Final payer adjudication determines the patient’s responsibility. If the policy, visit or evidence changes, create and review a fresh estimate.

Prepare an explanation or appeal draft

Under Evidence-linked assistance, choose the draft type and source response or claim evidence. Select the supporting material, review the draft, and save it for review.

Use View source to inspect the cited evidence. An authorized reviewer can Approve reviewed draft. Saving or approving a draft does not submit an appeal, change coverage or post a payment.

Resolve common problems

Use the recorded state to choose the next action.

Insurance or an action is missing
  1. Confirm the workspace.

Insurance must be enabled for that workspace. Switch back to the intended workspace if you have more than one.

  2. Check your role and environment.

Owners and administrators manage connections, submission and reconciliation. Editors and staff prepare work. Legacy results are read-only.

  3. Escalate the access issue.

Ask your workspace administrator to confirm the feature and your current membership.

A payer cannot be checked electronically
  1. Review setup and the payer message.

Check the connection and current payer directory. Some transactions require payer enrollment or manual verification.

  2. Confirm the payer identifier.

The identifier must match the selected payer and the transaction you are requesting.

  3. Obtain a documented answer.

Use the payer’s accepted verification channel and retain its reference, date and source.

A coverage result is stale, mixed or unknown
  1. Review the request context.

Confirm the service date, policy, subscriber, rendering provider and location.

  2. Read the individual benefit rows.

Check the service, network, dates and payer messages. An empty value does not mean zero.

  3. Resolve the missing information.

Correct changed details and request current coverage where the app permits it, or obtain documented manual verification.

A claim submission has an unknown outcome
  1. Keep the existing submission record.

A network timeout can occur after the clearinghouse receives an upload.

  2. Reconcile the receipt.

Ask the administrator to poll for responses and compare the account and submission references in the clearinghouse.

  3. Escalate unresolved evidence.

Do not start a new claim to bypass the unresolved attempt. Retain the request ID, time, environment and sanitized failure message for support.

A claim was rejected or denied
  1. Identify the source and reason.

Read the payer event or remittance, including its codes and timestamp.

  2. Choose the payer’s required correction path.

Correct rejected details through a new reviewed draft. Replacement and void workflows require the payer claim control number. Use the hosted appeal workflow when appropriate.

  3. Review the new submission.

Check the changed fields and current preview before submitting. Track the resulting payer evidence.

A remittance cannot be posted
  1. Resolve the exceptions.

Check claim and service-line matching, payment totals and adjustments. Ambiguous matches stay unposted.

  2. Verify the payment reference.

Confirm the EFT or check against bank evidence and the remittance.

  3. Request billing review.

Escalate unmatched claims, unbalanced advice or unsupported provider adjustments with the remittance reference.

An estimate cannot be approved
  1. Review the visit and evidence.

The eligibility response must apply to the current policy, provider, location, service and date.

  2. Resolve the calculation inputs.

Provide an allowed amount source, verified network evidence and applicable benefit rows.

  3. Create a fresh estimate after changes.

A policy or source change invalidates the earlier approval context. Test-account estimates cannot be recorded as patient deliveries.

When you contact support

Include the workspace, environment, affected screen, request or claim reference, time and visible error. Keep account keys, full member IDs and clinical documents out of general support messages.
